    Entry Permission:

    I/We require 24 hours notice before maintenance staff enters my suite to perform the maintenance.

    I/We hereby give my/our permission for the landlord or landlord's repair/maintenance person to enter my/our suite to inspect or carry out the above requested repair(s), between the hours of 8:00 am and 5:00 pm.

    I/We hereby give my/our permission for the landlord or landlord's repair/maintenance person to enter my/our suite in accordance with the 'Special Instructions' section below.

    This request is for a common space, and does not require entry to my suite.
